Enterprise made history again with its first discovery of a pre-industrial society much like Earth of centuries ago, but what began as a mission of observation turned into one of intervention. Captain Archer's plan to personally visit the civilization of half a billion humanoids called the Akaali took on urgency when neutrino emissions were detected on the surface, indicating the presence of an antimatter reactor that was obviously incongruous to this world. Disguised as the locals, Archer led an away team to the surface to find the source of these emissions, which they pinpointed underneath a small curio shop. They encountered a neighboring apothecary named Riaan, who talked about "evening deliveries" associated with the shop that she believed were responsible for a recent epidemic.
Archer and Commander Tucker later confronted the shop owner, named Garos, who they found was also an alien visitor. Garos, a Malurian, claimed the antimatter reactor was nothing more than a fabrication device for food and clothing, and the plague was caused by an indigenous virus. But samples of the local groundwater revealed contamination by a toxic industrial lubricant that was also out of place there. Archer joined Riaan in observing an "evening delivery" and discovered that materials were being transferred to an alien ship. Coming to grips with the knowledge that spacemen were visiting her world, Riaan joined Archer in breaking into the structure underneath the curio shop, which turned out to be a mining operation to extract a veridium isotope used to manufacture explosives. As Archer set out to disengage the dampening field around the mine so that Commander Tucker could transport the antimatter reactor away, Garos and his people attacked. Enterprise also came under attack by a Malurian ship, but once the reactor was transported into space, the crew fired a torpedo at it and caused an antimatter explosion that disabled the hostile ship.
On the ground, Riaan helped Archer overcome Garos and his henchmen, and the Malurians were forced to leave the planet. The Enterprise crew removed all the mining equipment and provided an antidote for the plague, all without the Akaali's knowledge (except for Riaan, who pledged to keep the incident a secret).
